We will build on existing partnerships between the council, the community sector, and the NHS to ensure more timely discharges for patients from hospital and wherever possible moving to their own homes.
We will include expand our reablement support to increase the number of people supported to regain or maintain their independence.
We will continually review our practices, so people receive the right support, at the right time, in the right setting to help them recover quickly or prevent their needs from increasing.
We will continue to work with health partners across Cheshire and Merseyside to raise awareness of the issue of limited NHS dentistry places.
We will continue to work with the NHS to safeguard and enhance services available at Victoria Infirmary Northwich, Leighton, Wirral Hosptial Trust and The Countess of Chester Hospital.
